8 Evaluation and assessment
As the program comes to an end, there are a few ways to assess whether it has met its goals. In the inaugural program run-through, the Program Manager sent out assessment surveys to the Champions and mentors, and also ran 1-1 interviews with Champions and a focus group with the mentors. There are example documents and templates below to send out to champions and mentors, and to run the interviews and focus groups.

The Program Manager also wrote a grant report for CZI, which details the motivations and outcomes of the program.
